Think more in terms of prevention rather than treatment. What can young people do to avoid obesity? How can they avoid it? Healthy habits, etc. Then start thinking about the message on the poster, AND (guess what?) since they are a young group, you can think of design that appeals to them :)

i guess there is so much to say when it comes to obesity.
As nicos highlighted you need something to appeal to the viewers and affect them..
